If you receive your ESA50 after 28/01/13, it "MAY" differ from the PDF ESA50 03/11 that can be downloaded from here :

Using The PDF Version Of The ESA50 Questionnaire

If you have already started filling in the PDF ESA50, or you intend to use the PDF version, check that it is the same version as the one that you receive through the post.

UPDATE- ESA50 03/11 form arrived in post today 25/01/2013.It had been posted to me by 1st class post with letter attached-dated 23/01/013.The completed form must be returned to Atos no later than 25/02/2013.Atos Healthcare Wembley MSC PO Box 1044 London SW95 9EB
As I have indicated in earlier posts I have for some time been preparing draft using a print of ESA 50 form from link on site.In addittion I have sinificantly referred to Guides from site to help me.I will have to write by hand as I am a one finger typist.
I also have a 01/2013 medical assessment report written by my GP with this migration process in mind to attatch to completed form.
I will update you when i submit my ESA 50 form.

I am glad to see that you have checked that it is ESA50 03/11, as with the PDF version.

Hi Progressing well so far with completion of ESA 50 RE-process Question-Should I send copy of ESA50 to Decision Maker at DWP-if so -Which office?? I recall comments from previous posts-is there any update to these ??
I phoned ATOS and asked if they could tell me which DWP centre they will send there advice to-they told me they will send to DWP Benefit sorting office and DWP will send to J C Plus office dealing with my claim!!! I contacted DWP call centre 0845 6088583 who put me through to Team dealing with migration IB To ESA-they asked for some personal details and told me it would be DWP Benefit Centre for my Area-Watford-which happens to be the DWP J C Plus office that has historically dealth with my I B claims.-To be honest I was not left feeling confident that this was correct advice-but that is it as far as it goes.Do not want to burn up to much nervous energy on this-just trying to get things right-ANY Comments would be welcome.Thanks
