Hi all,

Long story short, got a letter this morning from job centre plus to say that they would like to see me late next week to complete a form for ESA as i will no longer be entitled to income support as of 23rd July.

As i do not want to be rushed in to filling out this form i have requested an ESA form from them however they are saying that they do not keep forms at their office and that it is all done on computer, sounds strange to me. Anyway does anyone know where i might be able to get a claim pack that i can go through before my meeting with them next week.

Not sure what form you are being asked to complete, but the ESA claim form is an ESA1 which is available on the following link.
