How do you request your medical to be recorded

What is the procedure for this request and is there a template for us to use

You can make the request on your ESA50 or if you have already returned this, write to ATOS, with a copy to the DWP, requesting that your medical on date xxx be recorded. There is no template letter for this.

Just to add to what Gordon has said - there used to be (and may still be) a template letter on this site. However, now that DWP have agreed that recording is possible, the letter is largely redundant as a simple request should suffice.

This query is never ending and takes up moderators time. All posts on recording medicals can be found by using the easy to use 'Search Forum' facility.

Type 'Recording Medicals' in the 'Search' brings up 35 different threads on this subject.

I personally will delete all further posts asking for information/advice on this subject since it's already been replied to in the region of 250 times.

I suggest that other moderators do the same, but of course that is your decision.
