You should receive the ESA50 within 2 weeks of the phone call, so if you have not, I would start by contacting the DWP to let them know this, they may well refer you onto ATOS to find out where the form is.
It may well be that despatch has been delayed due to problems with the system in general, but it is important to find out what is happening as your claim can be closed if the ESA50 is not returned.
If the ESA50 has been issued and you have not received it, it is important that you confrim this in writing to the DWP as soon as possible, to avoid problems at a later date.
