ESA50 additional pages

On the ESA50 form would it be better to staple additional pages to each section of the form,or should I staple them all at the back and number them.Also my doctor is on holiday at the moment and my form will need to be sent back next week should I ask them to make sure they contact my doctor for a medical report.Thanks.

Hi stu,

You can attach extra pages to your ESA50 in which ever way is best.

Always put your Name, NI Number, and Date of Birth, on every extra page, and it may help to number them 1 of 3, 2 of 3, 3 of 3, etc, etc.

You could also draw reference to the extra pages in the boxes in the ESA50, e.g. please see : page ? of ?.

You can request that they contact your G.P. , but there is no guarantee that they will do this.

You can send off the ESA50 by the deadline, and then post on any supporting evidence from your G.P. to the same address, as soon as you have it.

Thanks for that, got a doctors appointment 6 august might just manage to send additional evidence to them.

O.K. stu,

It may be advisable to send your ESA50 by recorded delivery, keeping a copy.

If you have to send any supporting evidence from your G.P. after posting off your ESA50, do the same, and put in a covering letter stating that you wish this extra evidence to be taken into account with your ESA50 when assessing you.
