Hi

I am new to this site and am starting to try and read the great guides and advice on offer. I am currently being transferred to ESA from IB, which I have been on now for some years. Following the advice I already read I have made an appointment with my GP next week, she is very good and has been both sympathetic and very supportive.My form is due back on August 14th so I don't have a great deal of time.

If I asked her to write me any additional medical evidence would she charge or will the DWP simply write to her and ask her about my medical history. I have a letter from my consultant which has confirmed my diagnosis, however this is 3 years old. I will send it anyway but would really like to have some current supporting medical evidence.

I no longer attend the Pain Clinic so will not be able to ask them but I am very mindful of the need to send in as much supporting evidence as possible and am not sure who to approach. Thanks.

Welcome to B&W, if you haven't seen it already you might want to have a look at the following FAQ

It is at your GP's discretion whether they will supply you with a letter, and whether they will charge you for doing so.

It is possible that the DWP will contact your GP for further information, but they are not required to.

Please refer to either page 62 in the guide "Employment & Support Allowance claims on Physical Health Grounds" or page 64 of the guide entitled "Employment & Support Allowance claims for Mental Health & Learning Difficulties" which deal directly with the question of supporting evidence. See:-

Thanks to Gordon and Crazy Diamond for their reply. I have to admit I keep mulling over what to put, I feel I am in danger of writing an essay, I am trying to remember all the treatments and procedures I have had done, especially those that were unsuccessful.

And every time I think about the whole business I get a horrible panicky feeling. I was lucky when my IB claim was up for renewal last time as I didn't need a medical and my claim wasn't up for re-assessment until April 2014 so I feel a bit cheated that this has come along now as I suppose I was hoping I would be "safe" for another 2 years.

I want to be as comprehensive as possible about all my problems and the difficulties they cause me but I don't want to waffle or bring up old problems that perhaps are not relevant, even though some are still ongoing. It's more a case of knowing what to leave out than what to include.

Hi I am also new to site madcap woman and have just sent forms away 3weeks ago. I could not fit everything in I wanted to say in the use this space so I stapled a4 paper to and headed it with what question was being asked and i national insurance number and my name I wanted to give lots of information about how my disease affects me good luck
