Medical Reports for ESA was created by Tink60

Hello
I am currently filling in my ESA form. I was medically retired from nursing in 1993 and have been on ICB since then.
I have never sent any medical reports in, and when I have asked my GP, DWP have never written to him either.
I am going to send in my form, as it is due back next week without any medical reports. Has anyone does this ?

Hi T60,

Have you used our : ESA Claims Guides to fill the ESA50 in ?

If not, I would strongly advise that you review the Guides ASAP, as they point out any potential pitfalls in filling in the ESA50.

Have you considered : Using The PDF Version Of The ESA50 Questionnaire

I am not personally aware of any members who have sent in the ESA50 without any extra evidence. However, that does not mean that non have.

Yes, my first ESA50 was submitted without any accompanying evidence and I was placed in the Support Group for six months without a medical assessment. I think I am an exception rather than a rule to this though, and as an intercalating full-time student in receipt of DLA I am automatically treated as having Limited Capability for Work, so this could explain it.

I imagine the vast majority of claimants do not send any accompanying evidence.

I sent ESA50 without evidence when I was being migrated from IB toESA. I did this due to misleading info from ATOS in the preliminary phone call when I was informed that I should fill in and return the form. I would be called to face to face assessment after my GP had been contacted to give a report of my medical conditions. This did not happen. I was placed in WRAG, and I am now in the process of a lengthy, stressful, tedious appeal, despite having provided information on the form relating to meeting at least 2 of the support group descriptors.

I wish I had known about the Benefits and Work Guides earlier. In future I would follow them to the letter. Send as much evidence as possible. Be as creative as possible with any information you can think of. For example, for my appeal I got access to my GP records. Old reports are proof of my long term conditions. I have used a referral to the wheelchair service and subsequent records of hire of power chairs and mobility scooters from the local shop mobility charity, statements from carer and others who know me etc. there is often evidence to support a claim in unexpected sources. Just need to think calmly and rationally, even though the process is stressful.

Best of luck, keep calm and find evidence.
