Where to send Medical certificates

Hi all
Does anyone know where i send my medical certificates to.
I failed my ESA medical and Im in the process of appealing, do i send my medical certificate to the same place as my appeal form?

Also Gordon,i know its advisable to send recorded delivery if its possible,can you use the pre-paid envelopes that jobcentre plus send you when your sick note's about to run out and just pay the difference between the pre-paid part(usually 2nd class so worth 50p at the moment)and cost of 1st class recorded(£1.55 for normal sized letters)so basically just adding £1.05 to the pre-paid part? thanks

The Post Office allowed me to do this in similar circumstances.
