Hello forum
I am compiling my SAR list at present, so far I have asked for esa55, esa85A, LT54 forms, I am a little unshure if the LT54 is infact the points score sheet or maybe the score sheet is a seperate item ? any advice on that one.

I have listed all the other related evidence as per the B&W appeals section on page 12/13

A question that I am wondering about, did the HCP who made a paper decision (no face to face) on my conditions make the score sheet up in order to see how many points I obtained OR is this done solely by the DM when atos send back there report on me to the dwp ?

Finally, when sending the SAR request is there any department that deals with SAR's ?

Hi Dq,

You can put a Subject Access Request under Data Prorection Act 1998 in the form of a letter, you can also use the DWP SAR Form as here :

(I am not sure if I have given you this info before. :silly: )

You will be very lucky if there is a score sheet, and the ESA85A's are most vague.

There should be an LT54, there may be an : ESA72 Form and an : ESA 113 Questionnaire if they sent one to your G.P. for completion.

To cover all bases, you may wish to list the ESA forms that you are requesting, and then state that you are also requesting any forms, documentation or evidence that was used in your ESA WCA, hard copy or electronic.

If you put SAR Data Protection Request on the top left of the envelope, this should suffice, it should be done as a seperate issue to your appeal and sent in a seperate envelope to the address of the Benefits Office that is dealing with your ESA.

Thanks for the reply, all info's duly noted.

This "score sheet" there must be one as how would they know how many points and in which descriptors I scored them in ? to have come to a decision to place me in wrag I must have gained enough points.

This is the document I really want to see, as from there I can possibly make further decisions.

Has anyone on the forum ever got there score sheet sent to them when applied for ? would be interisted to hear.

Hi Dq,

I cannot remember any members posting that they have obtained a "Score Sheet" if they have been given an award as a result of "Paper Scrutiny" and an ESA85A has been compiled.

The ESA85A generally states what descriptors you don't fulfill and states things such as "Clearly meets the threshold of having LCW". It is full of LiMA generated phrases.

They are generally a pretty useless piece of paper really.

I am sure that members will post otherwise, if applicable, but don't hold your breath.
