My ESA was terminated on 4/12/12 as I scored 0 points on my medical with ATOS. I am appealing against ESA decision and my ESA has been reinstated from 4/12/12 at reduced rate of £111.45 pw. I had spine surgery following years of pain in the whole of the spine, particularly in the lower spine and PLIF surgery was carried out in Sept. 2008. I was released from the hospital in 2 days and was denied any help with physiotherapy or even the basic help such as changing of the plaster covering the surgery wound. Since then, I am left with even worse pain than I was prior to the surgery.
Standing, sitting, walking, sleeping, having bath/shower is all painful. Driving car is almost impossible. I am receiving DLA highest mobility and lowest care components. It has been long since I was released from the hospital and also substantial time since I was seen by social workers in Harrow to assess my needs. I feel, I am severely short on current evidence to prove that I actually have physical difficulties. How and where from do I gather written evidence to prove that I have difficulties. I am also under medication for hypertension and hypoglycemia. I severely lack control over my bladder and often end up discharging urine whilst away from home (e.g. to attend GP surgery) which is preventing me from making attempts to leave home. This happens even at home at times especially early in the morning when i am unable to control my bladder and end up wetting clothes/carpets etc. My hands/legs are always shaking and carrying a mug of tea from kitchen to living room (about 10 steps or less) makes my hands/legs shake and end up spilling some tea on the floor. Can anyone please help?

You can approach your GP and Consultants to write reports on your condition(s), however, they are not obliged to provide you with this service and even if they do, may charge you.

You can request a copy of your medical records from your GP practice, most consultants will write to your GP after they have seen you and these should be in these records. See
