ESA113 respose time by GP was created by Tao
Hi, is there a time limit within which a G.P has to return the ESA113 form to Atos?
I ask this because my GP retired recently and my new GP , who I've never met, understandably wants me to attend the surgery so that she can understand my problems and fill out the form accurately (I'm assuming that it is an ESA113 that Atos sent the surgery). Trouble is the appointment isn't until 19th March as GP is on holiday. Would this having to wait 3 weeks or so be a problem?
I'm being migrated from I.B to ESA (hopefully). I returned my ESA50 to Atos by its due date of 11th February.
Thanks for your time

I am not aware of any timescale, but it may well help your claim if you contact ATOS and explain the situation.

Hi T,

This is an ESA113 Questionnaire :

ESA 113 Questionnaire

It appears that it should be returned filled in, within 5 days ?

See : "2.2 ESA113" Page 6 of this document :

Thanks for your response, Gordon & Bro.

Yes, I'm only assuming that is an ESA113 that Atos sent my new GP. When the surgery phoned me they suggested I see my new GP because Atos had written to them requesting information and I haven't been in touch with the surgery for some years to update them of my condition. Are there any other official forms that Atos send to GP's regarding collecting further patient information?

Hi T,

The ESA113 Questionnaire is the only one that we are aware of with ESA.

There are also G.P. Factual Reports, (GPFR) but these are generally used with DLA, as you will see in the document that I gave you the link to.
