What happens to Doctor's Notes after Home Visit

Does anyone know when you make a request for information held by ATOS or DLA if it is possible to get a copy of the actual notes that the ATOS Doctor makes while undertaking home visit medical? I am not referring to the actual form that DLA receive from the Doctor.

Hi d,

I am not quite sure what you mean by ""notes" but you should be able to gain copies of all documentation written by The ATOS "Doctor" that carries out a Home Medical for DLA.

You can do this by issuing a Subject Access Request (SAR) to The DWP, not ATOS.

Thanks for reply.

While at my house Doctor wrote down notes on a form. I take it the Doctor uses these notes to complete the form he finally completes to send into DLA Office as most of the information has been omitted.

Oh I also found from the result of this home visit eyesight assessment that I am blind, I drive and I don't wear glasses!

As far as I am aware the visiting EMP (Doctor) would be writing directly on to The DLA Medical Report, if you feel that there is documentation that you have not been provided with, you can request "All" documentation used by The DWP DM, in coming to The DLA Decision.

I take it that you are not in fact blind and have no visual limitations.

If this is the case, and you have this erroneous statement in writing, you can challenge the quality and accuracy of the document that it was written on.

Hi bro58

I was asked lots of questions which required specific answers which Doctor wrote down, however none of this information is on the for Doctor submitted. I did ask DWP for all documentation but these notes have not been received. It concerns me also as what happened to that document that contains so much of my personal information.

Yes you are correct, I have no visual limitations at all for distance and never have - regular eyetests and recent printed results confirm this. This is just one of many points that will be challenged, there are a vast number of them and so obvious.
