Hi m1983,

If you are going to add separate A4 sheets to an existing document such as an ESA50, GL24, etc, make reference to the separate sheets in the main document. e.g. "Continued on separate sheet page number 2, or 1 of 3, etc etc.

So you are compiling the whole thing as a "file" that is easy to reference, it can also help to write up an index, and place it at the front. e.g. Page 1 : xxxxx, Page 2 : xxxxx, Page 3; xxxxx, etc, etc.

The easier you make it to read and refer to, the better.

ok i have filled in the questionnaire also put my name,dob, NI Number, and a reference code on the evidence sheets i have photocopied, on some of the questions i do meet the support criteria quite a few actually

so what do i do now just post it special delivery and wait for a reply?
