new esa50 arrived

I was transferred from IB to esa wrag 1st sept 2011, I appealed and after 9 month nightmare was moved to support group after 10 minutes at the tribunal. expected the esa 50 in juneish so guess they decided to torture me again a bit sooner.
question is the form came in a white envelope with atos on the back and esa50 printed on the front... plus the return envelope is also white and the address is flowers hill bristol, also totaly different from last time. The letter with it says as I am applying or being transfered to esa ??????? they have sent me this form. just usual inefficiancy do you think? sorry about spelling I am in a state already and brain fog gets worse the more stressed I get yes i am paraniod I'm afraid but should I be worried? puzzled? sigh... already terrified, don't know if I can go through a repeat of last time it pushed me close to quiting life... well this merry go round is not life just nightmare repeatedly.

We have some members who have received such a form in error. You should contact the DWP and check whether this has been sent in error. However, unless you get a written statement from DWP saying that you do NOT need to fill this in, you should get it completed and submitted as requested.

thank you Mrs HB. Who would I contact? everything has the atos at flowers hill bristol address on it. I know there is an email for Atos but not DWP and I cannot use the phone
