Hi ad,

Just to add to MHB's good advice, you may wish to consider using The PDF version of The ESA50 01/13, which can be downloaded and saved to your P.C. from the link on this page :

Using The PDF Version Of The ESA50 Questionnaire

Whilst referring to the appropriate guides under the heading of, :
"New ESA50 form" on the page that MHB provided a link to.

From our : ESA FAQ’s

If you are being transferred over to ESA from another benefit, you may also find the information in our : IB, IS, SDA, Migration to ESA FAQ’s helpful.

Finally, see : Sending Documents to the DWP or ATOS

Hi

I also have epilepsy when I filled my form in I made sure I gave detailed information about what happens in my seizures and also how often I have them.
My partner wrote a supporting letter as well explaining what he sees on a daily basis living with me and how not just my epilepsy but my disability affect me. Because I have no memory of what happens in my seizures I can't really describe them I have to rely on what other people tell me about them.

So if you aren't aware of what happens in your seizures ask someone who has witnessed your seizures to write something for you describing what happens etc.

Hope this helps you a bit with the form. Good luck
