You could just do a SAR request for all the info if you don't mind waiting a few weeks?

{Still not received any of the above, extra forms LT54 etc}

They are on track!! proof large white envelope through the door Saturday.

Anyways the Internet ESA 04/15 i noticed will only except so many words
am i doing something wrong?

i did by chance find a older version i had downloaded for my 1st ESA claim transfer and that allows as much script as is put in.. can not see any code on it, though it appears to be the same wording as the above ESA 04/15 not fully compared both as yet... far to busy answering descriptors.

If i am not doing anything wrong, the boxes keep missing 1-3 extra lines i need to put in... page 17 and a separate piece of paper still will not be enough,
so could i attach a separate piece to each page where the boxes require extra room for a few more lines..
i was thinking of printing out the answers and stapling to each page with name national insurance etc.

Hi c6,

I am afraid that you must use the most current version of The PDF ESA50 04/15 !!

Yes, this version is smaller, and it allows less text in the text boxes.

I recently managed to copy and paste information over from an older PDF ESA50 to the new one for a friend with some amendments to make it current.

I had to re-format it so that the information that was in the text boxes in the older ESA50 that would not fit into the text boxes on the 04/15 PDF ESA50 was copy and pasted on to added A4 pages.

Or, you could simply type in each text box, "Please see my response to this question on added A4 Page xx of xx" then type your whole response on the added A4's for each question.

This will make it easier to copy and paste and amend at your next reassessment.

We advise that you attach all added pages to the back of the ESA50, with : Treasury Tags through a punched hole, or something similar.

You could also staple, dependent on how many pages there are, but do not damage or alter the format of the actual ESA50.

I generally type up an Index to contents and place that at the front as well !!

See : Using The PDF Version Of The ESA50 Questionnaire

^^ Not a clue but i will take a look

^^ will probarly end up having to do this.. And will get some tags.

^^ Can you show me a sample of an index please.. feeling thick

Can't get the Gp's admin regarding medical records to move any quicker,
I have nearly all corresponding medical evidence to my answers for the descriptors,
what with all the changes i want to make sure ESA overlapping descriptors to PIP are covered and vise versa :-)
So i am not rambling on but giving precise supporting answers on the ESA form.

Just keep it simple! As an example

page ... Contents
1 - Covering letter
2 - ESA50
21 - Answer for question (1) Moving around and using steps
22 - Answer for question (2) Standing and sitting
.
.
.
30 - Evidence in support of Moving around and using steps

Hi bro58
How to explain problems with: 8. Getting around safely
I understand is just for visual impairment

^^Can the ESA descriptor be answered if medications cause visual problems or there are minor eye problems e.g wearing spectacles.

And can physical and mental problems of getting around be incorporated into this descriptor. without visual impaitment.

The over lap of pip concerns me , ‘Planning and following journeys’.
In pip is that physical or mental or a mixture.
