It is perfectly OK for you to write or type your answers and attach them as extra pages to the PIP2, just make sure that each page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.

Attach the pages to the back of the PIP2 using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

Yes - as long as your answer is easy to read and understand. Otherwise you should use a supplementary answer sheet.

Also, I cannot use public transport, the question is - do you need help using public transport, answers are yes , no , sometimes. Can I not tick and write nxt to it that I cannot use at all. Or do they always want a box ticked?

We always suggest that you do not tick a box if none of them accurately reflect your situation. Leave the tick boxes empty and write in the text box something like 'none of these apply to me, see page x' then explain your problems on the supplementary sheets.

Can people get awards using solely GP notes as evidence, I am still not sure about sending in my old hospital letters, because of the way they say no damage shown in tests although they refer to my symptoms in detail. If GP notes can be sufficient evidence , then maybe I should focus on those.
1)Can GP notes alone serve as evidence to gain an award?

2)Will the assessors wonder why hospital letters are not included? (cos the GP notes will refer to hospital referrals)
