Medical factual report , what do i ask to put in

nah , no point upsetting them rather have them onside , as i cant force him to write me a medical report , although dissapointing i just have to be satisfied that if anyone does contact him from dwp or atos etc he now knows EVERTHING about me which as i said before i dont tend to tell him about my mental health problems especially in a 10 min slot, i only go to gp when i have physical problems , sorry to ramble i hope this is of use to someone , rather than it seeming like im having bit of chat .
