I am assuming it is to do with pip because I can't think what else it could be for but my claim is already with a case manager, is it too late for this form to be done and sent back to the dwp?
My appointment is at 15:50 today, not sure why he needs me there with him unless it's because he isn't my regular Dr as he has left the surgery and so doesn't know fully how my mental health affects me daily?

My pip assessment was booked in for the 8th December which the nurse cancelled and did a paper claim.
If she did a paper claim why would they send something dated the 11th?
My Dr didn't write down everything I said either, he ticked a box saying I only travel by taxi but didn't write down that I can only go to my Dr and I take lorazepam before I go to try to calm me down.
In fact he didn't write much at all, he put down all my medical problems and wrote down my medication but left off the lorazepam and ziplicone which I wrote down on the pip2 form.
He didn't say much about how my problems affect me, I think he just put anxiety next to agoraphobia, but it is way worse than just anxiety.
I saw a little about how I could ask for the report that is used to make the decision but I have already requested the report that the nurse did when she did the paper claim.
He didn't tell me what he was writing, I was just able to see a little bit, plus his handwriting was terrible.
I thought he'd read through it with me
