I have severe depression/anxiety/agoraphobia, autoimmune thyroiditis, adrenal fatigue and have been suicidal. I have been trying for a year to get myself better. I can't cope with much mentally and I get plenty of brain fog with my many symptoms. I posted here the other day regarding my ESA
I recently made a claim for PIP and was immediately refused with 0 points awarded. I am busy reading up on this site as to how to set out my Mandatory Reconsideration. I intend to put together a report rather than a simple letter OR should I at this stage just put together a simple letter? My thought is that if I put something more detailed together in response to their report, then surely it would be more effective than possibly having to go to appeal?
I am going to pick at everything they said, correct areas that are simply wrong in the assessor's report and state my case. The first thing I will be saying is "how can your Assessor judge my day to day needs and capabilities from a one hour visit"
I am gathering as much medical evidence as I can. I have approached my former NHS therapist and requested a letter outlining my conditions and limitations. I am also doing the same with my private therapist. I have asked my GP for the same although I have heard nothing back from him as yet. As I am unable to get to the surgery due to my agoraphobia we usually email each other or my appointments are via the telephone.
I also have several letters from my former Endocrinologist that I can include along with an initial assessment report made by a private Psychiatrist that I only saw once, along with many private test results.
1. Is there anything else I should look to include?
2. Should I, in my report, suggest I be awarded ?? points on certain questions?

Although it is important to deal with the issues raised in the assessment report, you will only receive an award by showing that you meet the criteria for one so this should be the focus of your efforts for the MR.
I would sit down with your PIP2, the assessment report and our PIP Claim guide to make sure that you have shown that you meet the criteria to score points. Be aware that many of the Descriptors have multiple elements, all of which need to be met in order for you to score. Also, where applicable, make sure that you have made use of "reliably" and on the "majority of days.
I'm afraid the whole point of the assessment is to judge your day to needs and capabilities in an hour, so arguing against the process is not going to have any value.

In what context can I make use of the word reliably please?
I am reading examples on the appeals guide and noted in some examples that the claimant stipulates how many points they think they should be entitled to. Is that something I can do and if so, how many points do I think I should receive on each aspect? Or do I merely suggest I should be awarded points?

Have a look at the PIP Claim guide for a better understanding of what "reliably" means this should then allow you to make use of the relevant phrases in your answer on the PIP2.
There's no harm in your stating that you believe that you meet certain Descriptors, I wouldn't mention the associated points as the two are intrinsically linked, the important things are to make sure that you do meet the criteria and to explain why this is the case.

From the PIP Claim guide
“Therapeutic source” means parenteral or enteral tube feeding, using a rate-limiting device such as a delivery system or feed pump.
“Assistance” means physical intervention by another person and does not include speech.
