As I said in my previous post you cannot make a digital recording, this includes making a single recording to disk and then producing two copies from it, the recordings must be made directly to tape of CD as the assessment progresses.

hi thanks for the replies i didn't realise i had stirred it up so much, anyway i had no luck with my GP i don't blame him he says if they write he is happy to write back, however i have received a copy of the medical assessment from the day which is laughable to be honest,
however my my main concern is the evidence that they say the have or more worryingly lack of i.e i sent two hospital specialist letters and other items and not one is listed even though it was sent by special delivery with the correct markings ie name N.I number etc at the time of the original review form? is this right, also the time they said the assessment lasted was twice as long as i was even in the building? any suggestions about the evidence going missing? thanks

Have you submitted the MR request yet?

If you have then write to them again about the discrepancy in the evidence, include a copy of the reports with your letter.

If you haven't then include the information with your request. Remember you only have one month from the date of the Decision to request an MR.

hi Gordon thanks for the advice again, i don't mind others commenting if it helps, i got the mandatory reconsideration off in time and raised questions with regards to the lost evidence.
just one question though on the assessment report it states the date at the bottom for example 01.01.2017 and time for example 16.04 pm what relevance is the time as it not the time the assessment was held at? if this make any sense at all,

It shows a lack of professionalism and care in writing the report which you could argue extends to it's contents in general, but in reality it will likely be put down to clerical error. You would be best served concentrating on why you meet the criteria to score points as this is the best route to an award.

hi me again, just another quickie is it usual for the DWP to contact a person to ask if they sending any more evidence before they look at the mandatory reconsideration? and to tell me that i am close to the dead line? also would this person be the decision maker or another member of staff.
