I am his appointee also my sons mental health is not very good at moment he is off work again due to this
when completing the additional information for each question is it better to type the answer on a separate piece of paper or is it ok to hand write the answer in the box and on paper
I know people have mentioned about using a separate sheet but forms say complete a part in the booklet
What is best way also some of the answers relate to other questions therefore I am going to duplicate some answers so no points are missed would that be ok

It's entirely up to you, if your answers will fit in the available space on the form then I would recommend that you use the form, but if they do not then use an extra sheet.
Just make sure that each page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.
Attach the pages to the back of the PIP2 using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

Dreading a face to face as this will cause my son stress and anxiety at the moment

Be aware that the form will separated into individual pages when it is received so that it can be scanned, you want to avoid anything that makes that process more complicated than it need to be, so a single staple securing all added pages to the form will be OK, but multiple staples may cause problems.

One area of the form I find difficult and have read information on is the 2nd part traveling alone to unfamiliar places I see people are not getting points for this as the DWP do not consider the anxieties associated with this and for some people the COD associated with traveling on public transport causes stress and the need to use a taxi
Can you provide any advice on this or we're can I read further information regarding this
Thanks again

Go back and read the relevant descriptors carefully. They do not mention 'anxiety' but 'overwhelming emotional distress' (or similar wording) and this is the language you need to use when framing your reply.
