Ignore what the form says, you can send what you want with the form.
The ESA85 should be a good idea providing it identifies how you qualified, your previous post suggests that there was some confusion over this.
Yes, you can include your PIP report but you will need to explain its relevance, although there are areas of overlap between the two benefits, the criteria can be quite different.

Hi quick update, since sending in my form in the middle of December, I hadn't heard nothing, was waiting for the face to face appointment but heard nothing, then on Friday I checked my bank and noticed i was paid more money than usual, so I phoned dwp who told Me I had been put into the support group for 18 months, so didn't have to have a face to face which is a relief and first time I have been put in the support group, was always put in the wrag. Big thanks to site...
