I would email them for the address (physical or email) of the assessment centre that is managing your claim.

Thanks, I will do that.
Also thank you for supplying me with the link; without your input I'd have ended up sending them to completely the wrong place

I have the address for Maximus that I need to send my hospital papers to.
Do I need to send a covering letter with them?
If so, do you have or could you provide me with or link me to a rough template that I could use as a basis for a covering letter, as I haven't the first idea how to write one?
Many thanks in advance.

Yes you should send a covering letter. I'm afraid we do not have a template letter. The important thing is to make all pages with your full name and National Insurance Number.

I got the brown envelope today. I have been placed into the Support Group again, without having to face a medical assessment.
This should be good news, and I'm really hoping right now that it still will be, however yesterday evening I posted my hospital discharge papers to Maximus, along with a covering letter explaining what they were and asking that they be taken into consideration in relation to my ongoing ESA reassessment.
They were sent First Class, and so should be arriving with Maximus tomorrow.
I am pretty terrified right now that having sent them after the date on the decision letter I received from the DWP today, that this will trigger another ESA reassessment.
What will happen now? Will I be sent another ESA50 to complete and have to go through this whole terrifying ordeal again?

Gordon might have come across this situation before, so I'm sure he will comment if he has anything to add.
