I can't tell you what to write I have no idea what problems they have and importantly, how the effect them.

Write what you see. If you want to phrase it in ways that resonate with the criteria then look at the Claim guides and the other documents in the DLA section.

Only if it is relevant to their conditions, for example if they are unhappy talking to people they don't know well or even other family members then it would be something to cover, but as you are the Grandmother there will be an assumption that you are close to the child.

Helen, you might want to say briefly how often you see him and under what circumstances. Saying you live a few minutes away is not enough - some families live very close and never speak to each other! You might do anything from child minding one evening a week to running him to and from a special school every day and helping with therapy sessions. But that bit does need to be brief, because the form is about his needs and you don't want to distract the reader's attention from the main point.

What exactly are his mobility problems? Don't try to write a finished answer first time you put pen to paper. Start off by getting a writing pad, or some scrap paper - this is just notes to help you put together what needs to go down on the form. Write down everything that occurs to you - then go through and see of there's anything that you have seen for yourself that's missing.

Then have a cup of coffee and take a look at what you have written again. Cut out everything that's not relevant and all the repetition and anything which someone told you about, but you didn't see for yourself.

Then, as Gordon says, you need to take a careful look at the stuff in the B&W guide. They change the details of what they give benefits for, so you need to read that carefully for the benefit you are applying for - the rules for DLA and PIP (for example) are not the same when it comes to mobility. Now re-write what you have so that it matches the things that the DWP are looking at.

Then hand it over to the person who is filling in the form and ask them if what you have written seems helpful.
