Pip1, Made phone call but no written confirmation

Hello, This is the first time I have used a message board, so I hope I am doing it correctly.
I was invited to change from DLA to PIP. I completed the PIP 1 telephone application. I thought from your guide I would then receive a written form PIP 1 confirming the call. I have not received this, only the "How your disability affects you" form to complete. The form is 40 pages long includin the tear off bits, and has PIP.1003 written on the bottom right hand corner.
Has the PIP 2 form been superceeded?
I am concerned they might have overlooked sending me the completed PIP 1 form to sign.
Have they changed their policy please regarding this please?
Any help would be much appreciated.
