I think you might have misunderstood -

DWP sometimes send a form to the claimant's GP to gather more information. However, they only send this in around 25% of cases so you should not rely on it.

Many members ask their GP to provide a letter of support. There is no template for this, we often suggest that the member downloads a copy of the descriptors for the relevant benefit, remove the points score and give this to your GP with a request to comment about this as much as they can.

The descriptors can be found in the guides for whichever benefit you are claiming.
