PIP GPs '' Forms Letters and the Like!

Still not techy so sorry if not linked correctly thank you

I was at GPs yesterday for medical conditions and I referred to PIP (in general not for me specifically though I am concerned) and my concerns: ? Would they GPs write a letter etc

and was told by GP''
that (for another patient) yesterday GP recv'd 'Letter (?Form)' from PIP re said patient.

And it was series of can this patient do this that or other?: GP did not go into specifics of what the this and that was on form or letter PIp had sent them : however said that also some things that PIp asked (in the letter form sent to GP) could patient do or condition restrict:? whatever 'task'

GP had to say did not know the answer and said so.
I checked twice with GP that I ahd understood what GP had said correctly and reply was yes.

Q Does this sound correct and what is the specific form or letter sent o GPs :
I guess there is a template blank somewhere?
Is there anywhere to view? please

I recall there was something like a eg ned 3 form (not exact term)
when ESA asked Gp I had old copy tossed now and this is query on PIp: and if so form/letter number or reference please

(A slight PS) I am not sure if GP was ref to appeals' GP did not say appeals, however there was not the time to discuss or approp. really to discuss further, as number of med conditions for Doctor to attend to for me.)

The GP will have received a GP Factual Report, there is a section in the PIP Claim guide that covers this.

Unfortunately many GPs are only concerned with the medical aspects of their patients conditions and not the daily impact that these cause, many (mine for example), refuse to engage with their patient in a way that would allow the patient to explain how their conditions limit their ability to complete day to day activities.

I'm afraid I don't have a solution to this, even moving GP makes no guarantee that you will be better off.

A Med-3 is a Fit Note and is only issued if the patient is unable to work.

It's common for GP's to offer help if the claim goes to appeal but not before, quite how the justify the hurt and impact that this will certainly have on their patient I don't know.
