Welcome to the forum, you might want to have a look at the following FAQ which explain where everything is

Welcome to Benefits and Work

First of all have a look at the PIP Claim guide, there's a section at the back of the guide that covers the assessment and the questions you are likely to be asked.

AS there have been changes to your capabilities I would write a short letter for the assessor explaining what those changes are and how they effect your ability to complete the PIP activities. So you mention that you can no longer Prepare and Cook a meal, explain what has changed and why you are now no longer able to do this.

You can also tell them about the operation and what is being done.

Give this letter to the assessor when you see them and ask them to read it and include it in your file.

I would also send a copy directly to the DWP, c/o the Decision Maker, the address will be on the letter that came with your PIP2 form.

If you have further questions then please reply to this post and we will do our best to help.
