Hi I'm in ESA Support Group & have till month's end to post it off.

I'm seeing my GP first thing tomorrow (although reception told me they can no longer give me a letter as in the past). So apart from asking for an updated prescription list - - how best do I use this appointment? Thinking I'll brief her on latest symptoms ( I go at least bi-monthly) & let her know about this form? I'm taking along her previous letter and today's very comprehensive letter my retiring support worker has just updated for me as my conditions have worsened since the last form which she used to write for me.

I am paying a small fee to Diverse Abilities to come on Thursday morning to hand write the 22 page pink form for me. They've helped in the past and will go to appeal if needed. Hence why I've joined your site and downloaded your ESA fact sheets to help in wording together.

My support worker and Diverse Abilities both advise to get supporting evidence but on the form they repeatedly say do not ask for new letters use old???

I'm seeing my private physio on Friday - not sure how or what I can ask of him - only been six months so far.

Thank you so much for any tips :o)

It will depend on whether reception was right and your GP will not write a letter of support.

If they will then you ideally want them to write about the problems you would have completing the ESA activities, your GP may be unwilling to do this but they ought to be able to comment on some of them at least, based on your conditions.

If they won't write a letter then updating them on your problems is a good idea.

A letter from your physio would be of value if you can get one.
