Do i need to tear off the first 2 pages of the PIP form or just the first page please?

I honestly can't remember, but it does give clear instruction on the form.

PIP update and award. I have now received my award for PIP from DLA.
This is a timescale and brief description of how it all went.

Received letter dated 17th February 2017 stating DLA is ending and to apply for PIP by 17th March 2017.

15th March 2017 rang PIP and answered PIP1 form questions. 15 minutes later received text saying thank you for applying for PIP and PIP 2 form was in the post. (Very prompt)

25th March 2017 - Received PIP2 form dated 15th March to be filled in and sent back by 15th April 2017.

28th March 2017 - Rang DWP to request 2 week extension on when the form has to be received by them to the 29th April. Assured it would be logged on computer and new date deadline for DWP to receive ALL paperwork and form back is 29th April 2017.

30th March 2017 - rang PIP to double check the date on their system that is definitely logged for me to send everything back by 29th April 2017. It had been logged on their system as 28th not 29th so glad i checked.

25th April 2017 - Sent PIP form, evidence, GP report, GP home visit request etc in envelope guaranteed next day before 1pm.

27th April 2017 - Received text saying got pip form and will be in touch.

Thursday 8th June 2017 - Received letter saying have home visit on Monday 12th June at between 9am and 11am. Home visit was given straight away which i was very pleased about but didn't have a lot of notice but went with it anyway.

Monday 12th June 2017 - Home visit face to face medical assessment.
Time started - 9.05am
Time finished - 9.50am

Thursday 29th June 2017 - received council tax review letter stating review due to new DWP income.

I knew then that a decision had been made but wasn't brave enough to ring to find out the award.

Today - Postman came, heart beating out of chest and shaking at the brown envelope on the mat. Opened it and it was an ESA change letter due to new DWP income. So many other people been notified of award except me. Got brave and rang PIP to find out award.

Went from DLA high on both daily living and mobility indefinitely to PIP standard daily living and enhanced mobility for 4 years. Am happy with the result and will not be appealing as i have ESA tribunal looming already. I am just pleased its all over for another 4 years.

So start to finish it was approximately 4 months and 1 week. I could not have done this process without the wonderful guides, continued advice whenever needed and the forum. I literally put my everything into getting it right by following the members only guide to the letter. The subscription fee for this site is worth every single last penny and more. I cannot recommend it highly enough. Its still been worrying at times not knowing what is going to happen but i truly felt supported by this site.

I wish i had known about it before i had my ESA application as i got 0 points and found fit for work. Completely ridiculous and am now at appeal stage. Thankfully i have the wonderful guidance of this site to guide me through every step. Have created a separate topic for this and will keep it updated with the progress.

A huge, massive thank you to this site, all the moderators and the members who share their experiences on here.
