Filling out the new AR1 form was created by fastmoggy
Hi,
We have received the new AR1 form and just correlating all the additional evidence to help with my wife's review.
On page 18 regarding supporting information, they state they don't need any information more than 2 years old or appointment letters.
This I'm concerned at because on her last review the HP asked several questions regarding what appointments she had attended ,some more than 8 years old! and when, which we couldn't answer straight away as we wasn't expecting it even though it was at hand for the review.
So my question is, Should we just include all the appointments/consultants attended which has all the dates they requested at the interview? Also should we send them all the evidence from day 1 rather than information no more than 2 years old?
Thanks for any advice/experience regarding filling out this 'new' form

It's entirely up to you what evidence to include, they are supposed to review the evidence submitted with your previous PIP2, but we have doubts that this is being done to the level required.

I would be cautious about sending everything that you have previously but certainly would send diagnosis letters and ones that directly address the PIP activities.
