Report changed and assumptions was created by mb
I received my report today going from daily living and mobility award to nil.
Having read the report from Hp and MD they based my claim on letter from Gp and previous evidence in which I got the award. They have wrote that under the assumptions I do not meet the criteria.
They have also changed the descriptors I put to different ones on Hp form which I did not fill out as I had the pip2 form which just said any changes.
I have already requested a mr of descision
And gone through each descriptor.

Any pointers would be appreciated or advise

The most important thing to ascertain at this stage is whether the PIP2 from your previous assessment was available to the assessor, as the Review form only deals with the changes in your condition the previous assessment, it is essential that this form is reviewed with the previous PIP2.

It should list the documents used on page 1 of the PA4 assessment report.

The first stage to challenging a Decision is for you to request a Mandatory Reconsideration, this needs to be done in writing to the DWP, within one month of the Decision, to the office that dealt with your claim, have a look at our PIP MR & Appeal guide for details of the process, the PIP area also has template letters that you can use to make the request with.

Your primary task is to show that you meet the criteria, there are many reasons you may have failed, you need to address each of these but don't get bogged down in criticising the assessment report unless you can clearly show that it is incorrect, it is a lot easier to argue the facts of the situation;

"the assessor recorded that I walked 50m, I did but they have failed to document that I had to stop every 10m for a rest due to breathlessness"

than their opinions

"based on my observations of the claimant walking I believe that they can reliably walk more than 200m.

They have recorded on list of evidence that it was my origalpip form they used as well as my Gp report . My condition and how it affects me will not change and is variable.
I also was reading the report again and it doesn't match with the Hp what he's wrote under descriptors it just says claimant did not report any thing. But when looking at the questions i.e. No help with cooking nil. But he has wrote that I use preaching stool, dangerous when lifting pots due to numbness in fingers and pain. Needs someone to cook meal as she cannot do activity.
I have followed your guidance and I'm going through each descriptor.

It's almost as if two people have filled in the assessment report, one who only read your PIP2 and one who only read the Review form!

I usually advise members to concentrate on showing that they meet the criteria to score points but I think that this is a case where you should spend some time showing the differences between the two.

That said, you will only score points by explaining the problems that you have so don't lose track of doing this.
