Hi Gordon. Another query just come across is that hubbies doctor has just given him another sick note for 10 weeks. We have just realised that his 28 weeks will be up in about 4 weeks and then he has to apply for contribution base ESA. On the form it states that an up to date and original sick note has to be sent with the application. His original sick note will have been given to his employer (taking it tomorrow). We also take copies but will DWP accept this. Otherwise can anyone advise what we should do please.

A copy of the Fit Note given to his employer should be sufficient, the DWP should know that the GP can't issue an overlapping Fit Note only a contiguous one.

There isn't a single address, each DWP has a unique address but all at Wolverhampton, the letters you have received about your ESA should have the address on, failing that, have a look at the following.
