Pip

Hi I previously had a life award of Dla middle care and higher rate mobility I've received my PIP forms to complete however I'm confused is the PIP form to claim mobility as well it's 28 pages but I cannot see anywhere that I state my ability to walk ect ?
Is there another form ? Please advise

The last 2 questions on the form deal with planning + following a journey and walking. See the PIP guide for explanations of the questions and how you might answer them.
