Hello all,
Can anyone point me in the right direction to find information on what forms the Tribunal Service would send to a GP for 'medical records' information please?

Hi Wilf,

In my case the tribunal service sent a standard letter asking for my medical records with a request for my doctor to provide a brief factual report. I think its just a standard letter.

Hi, your question is a bit confusing as in the title you refer to the DWP to GP form but in the question ask about the tribunal to GP?

If it is in the case of the tribunal, as Kathy says above it would most likely be a general request letter asking for specific information as opposed to a form.

Thank you Kathy & Pete. Sorry for the confusion Pete but I wrongly thought that the Tribunal was part of the DWP.
My question is now answered I feel as it was the Clerk to the Tribunal who wrote to say they were awaiting my ''medical records'' from my GP.
Once again, many thanks to both of you for your help.
