How do I find out where to send additional evidence?

Hello

My first time posting here! I have tried to search the forum but can't find the exact answer easily. I apologise if this has been asked many times before.

I can't seem to find a note of which DWP office I have to send my form off to. It was a change of circumstance form for PIP. It's taken me awhile but I now have a letter from my GP and also a family support worker which I would like the DWP to consider full stop how do I find the correct address to send it to?

I am aware I need to put my National Insurance number and name etc on each piece of paper.

Welcome to the forum.

To send in any additional support documents use the address on the DWP letter, do not send original documents as you will not get them back, send a photocopy.

Any documents sent to DWP always send them by recorded delivery and as you have stated always add you name and NI number, you may want to add page numbers if sending more than one document.
