www.benefitsandwork.co.uk/guides-for-claimants/faq/forum The first you need to appeal is get a copy of the assessors report called a PA4, once you get the report you then can see what you have to appeal against.
We cannot help you with the appeal as we do not know what you want to appeal against. As you state there is a guide on how to appeal, you do not need to remember everything, you just need to follow the guide carefully.
