The address on the letter i have to send the GL24 to is in Glasgow, her local JCP which if she would have to sign on for example is in Wandsworth. i know i sound thick please forgive me, which do i send fit notes to ?

It may be worth ringing up to check first, make sure you keep a copy of the fit note, and if possible post it by recorded delivery.
The G.P. may backdate the fit note to the 18/11/11 on request, if the first available appointment is the 22/11/11.
