Hi hope you can help! After a 5 month wait I have finally been put in the support group of ESA. I believe I should also be getting the Severe Disability Premium of £55.30 a week (at the moment). I phoned the DWP this morning to be told that there`s another form to be filled in, he did tell me the number for this form, but unfortunately I can`t remember! Has anyone else had to fill in this form? How involved is it? Was the process a fairly simple one? Thanks, in advance for your help. Derek.

Yes, I meet the criteria in every way as set out by DWP.
Hi DB,
Disability premiums are paid in the form of an Income Related ESA "top up".
In effect the entitlement to a Disability Premium raises the weekly amount that the government states that you need to live on.
Therefore, payment of some or all of the Premiums will depend on any other income, assets or savings that you may have.
You may have to provide the DWP with proof of any other income etc, if applicable, as well as providing them with proof that you are in receipt of the necessary DLA award.
If however you have been migrated from IB, IS, or SDA to ESA, and were in receipt of these premiums before being migrated, you should receive exactly the same under "Transitional Protection".
