if the fact that you never attended a face to face medical which the DWP can trace with the assessment centre then I would request the MR with this being your first challenge to the whole sorry process or if you feel so strongly about this write to the DWP office who deal with your claim with a complaint, as you mentioned they have either mixed your claim with another, or done a paper assessment but worded the decision that you in fact had a face to face which you know to be untrue, I would also copy in your MP on this matter incase the DWP try to fob you off with another excuse like the so called phone interview where you could easy say nobody was with you at the time of the call and with great distress you answered the questions. My opinion is this should be set aside and the process started again
