in 2003 I took my DLA to an appeal tribunal. Awarded high rate mobility and low level care. Have had no problem in renewing my DLA applications to date. Now been requested to apply for PIP. In process of filling in the application form. My question is: Is their any good reason to make reference to the Tribunal decision in my PIP application? Also; I have to return the form by; 04/11/2016 --- what are the chances of me obtaining an extension to this date? Many thanks.

You would need to show that the Tribunal Decision relates to your conditions as they are now and that it is relevant to the PIP criteria, if you can do this then I would consider including it but if you cannot would it be anything more than a distraction from your other evidence?
Phone the PIP Claim line, it's the number you phoned to make the claim, most requests for an extension are allowed.
