hi do you have a pip assessment report if you didn't have a f2f. I only got my award letter a want a mandatory reconsideration but am unsure what to ask them for regarding their decision. thanks

Yes there will be a PA3 report, as opposed to the normal PA4.

DWP do not communicate by email. You can phone or write to request a copy of the form.

hi what address do I request it from, is it the address on my pip award letter or a different address, also is there a draft of his letter I could copy. thanks

The one on your award letter. I'm afraid there isn't a template, just write and ask for the assessment report, remember to include your NIN and address in the letter.
