I have been on ESA for quite a while due to a heart condition and fibromyalgia. I attended a medical and was placed in the Support Group. Now I have received a form, Capability For Work questionnaire. I'm not really sure why I have received this. I'm just really upset, I should have been claiming my pension by now, but the government changed my pension date twice now. I am now due to get my pension in 2019, and I am so worried that I will be forced to try to find a job as I have heard such terrible stories. I have until Dec 22nd to get the form back, can anyone give me some advice about filling the form in please.

Although ESA is an indefinite award all claimants should expect to be reassessed regularly.

This is a full reassessment of your claim so treat it as a new claim and complete the form in full and supply any relevant evidence as you have, even if it has been submitted before.
