What my husband and I find so odd is that the assessor knew it was recorded and yet still didn’t give an accurate account of what was said!

Interesting post and (the assessor knew it was being recorded) it's strikes me that they still and will even stoop this low and still lie in their reports?
It's a good job you have the evidence to back this up and quite rightly you can explain all the untruths within from the recording and as you've now submitted it into your MR. I hope they take notice if they don't im sure the Appeal will.
