Is it the report from the health professional I saw today I can request?
Just been told I get a report with the decision letter what is that one? Is it explaining the decision makers decision
Is it a good idea to phone and ask for the report so I have it for future reference

Yes, it's the report from your face to face.
The Decision Letter will have the Decision Maker's Statement of Reasons.
