You need to to inform the DWP that deals with your ESA claim, this should be where you sent your appeal, but be aware that another office may end up handling this, so you could need to correspond with two offices.

If there has been a clear and documented deterioration of your condition since the previous Decision, whether this is the result of the COPD or not, then you can request a SuperSession or re-assessment of your claim on this basis, this can run in parallel to your appeal, and may limit any award that a Tribunal may make.

Especially when dealing with 2 offices, possibly with 2 issues (and appeal and a supersession) it is very important that you keep copies of all the correspondence you have with both, and notes of any phone calls. Don't skimp on the photocopying of any doctors' evidence as you may find you need extra copies.
