Have the limitations on which your current award changed?

If not then you do not need to contact the DWP.

If they have then could you reasonably expect a different award if reassessed? If not and there has been a deterioration in your health then whilst I cannot tell you that you do not have to contact the DWP, the reality is that they won't be bothered if you do not.

If you have had an improvement, then you are under an obligation to contact the DWP.

If you think that your award would change then contact the DWP office dealing with your claim in writing notifying them of the change in your condition and ask to be reassessed.

Do you know if I would have to compete a whole new form?

If you report a change in your conditions then the DWP will reassess your claim in full, this will mean a new claim form and almost certainly a face to face assessment.
