Question 2A asks for your conditions only. You can explain any symptoms that they cause in Question 15, Additional Information if you have not covered them in questions 3-14.

No, they have no authority to do this, if they had any concerns then they might contact your GP, but even this is very unlikely. Whilst I have heard of assessors threatening to do this, I don't remember an occasion where they have.

Thank you for your replies Gordon. Very reassuring.

Conditions Vs Symptoms. Difficult one. They can be both the same. For example, although diagnosed with EUPD (etc) , I suffer many conditions as symptoms of my diagnosed illnesses. The NHS at www.nhs.uk/Conditions/Pages/hub.aspx list things like paranoia, agoraphobia, claustrophobia, social phobia, as conditions. I suppose if in doubt, list everything? I'd rather do that than miss something out which could be important.

In your position, I think I would write down the main conditions and then list the name of the problems it causes beneath each one.

Well, this is different, and from reading the forums here quite unusual.

I've received a letter saying

"We're writing to let you know that we've asked for further information from the people involved in your treatment or care"

"Once we've got the further information we've asked for, our Health professional may be able to finish your assessment report for DWP without having to see you in person. But if we don't get this information, or we still have questions, we might need to see you foir a face to face consultation."

With my application I enclosed a letter of support from my GP and one from my carer/friend. My Psychiatrist is totally useless and previously was only willing to supply me with a statement of my diagnosis, end of, despite me telling him it was how my symptoms affect my daily life which is important. He knows little about me or my issues because he's not interested.

My worry is that they'll ask my Psyc for 'further information' and that his report will go against me and contradict the report by my GP.

How can I find out which people they are requesting more info from and how can I find out what those people are submitting? I need to see a copy of what my Psyc may be sending in case it's a work of fiction.

Your problem is that the Assessment Provider is under no obligation to tell you who they are contacting!

My advice would be to contact your Psychiatrist's secretary to see if they are being contacted, you should also contact your GP and any other medical contacts that you included on your form.

If the Psychiatrist is contacted then it may be difficult as there will be an expectation that they will know what they are talking about in regard to your case, so you might want to think about how you might correct any inaccuracies there might be in their reply.
