PIP 1003 form and medical reports was created by Catherine
Hi, I'm new here and I have just received my PIP 1003 form. I am currently on DLA at the higher rate. In the accompanying notes, it mentions not to send reports from my GP etc but I thought I had read that these should be included. I have an appointment to see my GP on Wednesday and my Rheumatologist next Monday and was going to ask for a report/assessment as, like many others, I don't see either of them very often. I have (plenty of) previous reports on my Rheumatology clinics. Should I get the reports and include them or not? Thanks in advance...Catherine

It's your claim, you can send what you want.

Up to date evidence can make a big difference to your claim, try and get your Rheumatologist to comment on the PIP activities if they are willing.
